Per Diem RN Hospice

Per Diem RN Hospice for Sierra Vista and the surrounding area Are you a highly skilled and motivated Registered Nurse with experience in hospice care? Are you passionate about providing compassionate and high-quality care to patients and their families? If so, we have an immediate opportunity for you to join our dynamic and progressive team at Valor Hospice.
Section 1: Duties & Responsibilities Complete initial and ongoing assessments of patients and families to determine hospice needs Provide professional nursing care and develop care plans based on nursing diagnosis Administer medications and treatments as prescribed by physicians Assist patients with activities of daily living and provide health care instructions Coordinate patient care as a Case Manager Section 2: Communication Maintain accurate clinical notes and communicate changes in patient's condition to physicians Coordinate care plan with community health related persons Teach self-care techniques and provide health counseling to patients and families Ensure a safe environment for patients and provide continuity of care Attend interdisciplinary group meetings Section 3: Additional Duties Participate in on-call duties and ensure availability of necessary items and services Supervise ancillary personnel and maintain professional knowledge and skills through continuing education Contribute to quality assessment performance improvement teams and activities Location: Sierra Vista At Valor, we answer questions regarding care options, pain management, symptom management, expectations for care, loved ones' comfort and more.
