* A RN care coordinator is a registered nurse who organizes patient care and treatments by incorporating all members in the care team12.
They work with patients who have specific diagnoses that require care either inside or outside of the hospital12.
They provide care coordination services to patients, families, physicians, staff and payers using principles and techniques of case management, utilization management, and outcome measurement3.
They may also be responsible for delivery of care management in the long-term care and community setting as delegated in subcontracts with health plans4.
They assess and evaluate patient conditions, and then plan, implement, provide, and document patient care in a manner that follows professional nursing standards and outlined practices5.
* Will do Infection Control, Case mgmt and Unitization review Job Type: Contract Salary: $65.
