* *JOB DESCRIPTION* *SITE: *Mesquite Building, Mariposa Nogales North, Sierra, Rio Rico *JOB TITLE: *Behavioral Health Technician-2 (BHT) *REPORTS TO: *BH Program Manager PURPOSE OF JOB: Mariposa Community Health Center’s (MCHC) mission is to provide comprehensive primary and behavioral health care where the needs of the patient are always the primary concern of all staff.
The Behavioral Health Technician-2 (BHT) works collaboratively as a member of an interdisciplinary care team, serving patients receiving behavioral health services.
Essential Functions: 1.
Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patients receiving behavioral health services.
2.
Delivers health and wellness and recovery-oriented services to patients.
3.
Perform intakes, assessments, case management, and coordinates referrals and linkages into necessary health care and community services.
4.
Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
5.
Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate.
Coordinates care with a patient’s family, care team, and other involved supports including educational, social, judicial, and other agencies.
6.
Participates in staffing, huddle, case conference, or other meetings, with or without the patient and their supports.
7.
Provides excellent customer service to patients, internal and external provider agencies, State entities, or other stakeholders (e.
g.
probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.
) as needed.
8.
Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the client, input from the person and relevant others, and consultation with the clinical team, resulting in modification to the service plan as necessary.
9.
Ensures documentation in the Electronic Health Record is completed the same day Service Delivery is provided.
10.
Consistently meets Service Delivery expectations.
11.
Communicates and documents patient progress toward their recovery 12.
Embraces the recovery philosophy and promotes a safe recovery environment for clients.
13.
Uses brief clinical interventions, motivational interviewing, and supportive listening skills to engage and support patients receiving services.
14.
Participate in committees, staff meetings, and any meeting as requested by supervisor.
15.
Respond to patient or co-worker complaints and work toward a positive resolution of any dispute.
16.
Work under MCHC Polices and professional requirements.
17.
Works with MCHC Population Health programs to ensure compliance and adherence to quality measures.
18.
Perform patient transportation or home visits as needed.
19.
Maintain physical and mental health.
20.
Participate in, train, and utilize tele-health services in the evaluation, management, and treatment of patients, as appropriate.
Supervision: None Typical Physical Requirements: 1.
Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination.
2.
Provision of services may occur out of the office setting in other places of service.
Must be willing to travel to different locations for work.
3.
Standing and walking for extensive periods of time.
4.
Corrected vision and hearing within the normal range.
5.
Working under stressful conditions.
6.
Exposure to communicable disease and bodily fluids.
Knowledge Skills and abilities: 1.
Interpersonal skills which allow for professional and positive relationships with coworkers, patients, families, and other community members.
Good communication skills, evidenced by a nonjudgmental approach, effective listening and positive interactions.
2.
High level of comfort in working with multidisciplinary team using a consultative style.
3.
Ability to deliver high quality care to a wide variety of patient types, depending upon specialty training, and diverse individuals including LGBTQ, and all racial, ethnic, religious, cultural backgrounds.
4.
Interest in fostering patient cooperation toward the delivery of care.
5.
Strong understanding of substance use, addiction, and recovery principles.
6.
Excellent working knowledge of behavioral health and evidence-based treatments for medical and mental health conditions.
7.
Ability to make quick and accurate clinical assessments of mental and behavioral conditions.
8.
Ability to lead effective psycho-education classes.
9.
Ability to effectively represent MCHC’s interests in the community and maintain effective working relationships among coworkers, public, private, and professional groups.
10.
Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
11.
Ability and evidence of competence in general office procedures.
12.
Ability to employ the problem-oriented method of record keeping.
13.
Proficient in English language, written and spoken.
14.
Bilingual English/Spanish preferred.
15.
Ability to maintain accurate electronic medical records for documentation and billing purposes, NextGen experience preferred.
16.
Ability to conduct home visits and patient transportation.
17.
Possession of a valid Arizona Driver’s License, reliable transportation, and vehicle liability insurance.
EDUCATIONAL & Experience REQUIReMENTS: 1.
Bachelor’s degree in any field and at least one year behavioral health work experience or 2.
Master’s Degree or bachelor’s degree in a field related to Behavioral health or 3.
Licensed practical nurse with at least 3 years of behavioral health work experience or 4.
Physician assistant who is not working as a medical practitioner or 5.
Registered nurse 6.
CPR certification required.
7.
Must have a valid Arizona Fingerprint Clearance as a condition of the first day of employment.
